NPCs are individuals; NPCs have their own personalities. It is possible that the NPC has made a mistake, or that they have a particular psychological reason for their actions. A case like this is extremely likely to not be a GM error, but an In Character (IC) error.
If the player firmly believes that there has been a GM error then evidence should be sent to the GM explaining the situation, which will be looked at; it is very important that this is left as a last resort as GM time is limited and due to the nature of the game being full of misinformation many players could be convinced that a result has a GM error
